It really depends upon the local post office in Ukraine. One P.O. may be very good, and another nearby a cesspool of bureaucracy and incompetence. I have dealt with both. Using the English form of Name/Address/City/Country/ZIP is usually fine.

Generally, you have these three options using the post: 1) address it in English 2) address it in cyrillic, but make sure to add UKRAINE in english below the full cyrillic address (their ZIPs overlap some in the U.S.) 3) Fill out the pink card from the P.O. for a return receipt.

I have found that using the pink "Return Receipt for International Mail" is more likely to be received. You may need to ask at the P.O. for the pink card, they don't always have them readily available (the domestic version is green).
