: I sent for an application for a visa form back in April. I have just checked the Consulate's web page for Chicago. They state right there that they only accept original forms! No copies and no downloaded forms. You can download a form for an example of how to fill out only? In my case I sent a letter to them asking for the application. I sent it Federal Express. I also sent another Federal Express envelope along with it with my address information filled in and put it in the package with the request. I did not have any problems. They sent me the application back in this envelope and filled in the information needed for the senders part to. Total time was 1 week to receive it. Hope this helps. If you want I will email you a copy of the letter I sent.
