What you should do is to call Ukrainian Consulate in New York and ask them about all the details. Also you can try the Ukrainian Embassy in Washington DC. Usually they can fax the papers you need and then might require you to appear in person at the embassy. Good luck and be aware of the Ukrainian "strict" laws for us, Ukrainians in America.
