Do try again. It is hard to forecast the outcome, but keep trying. Chances roughly are 50/50. However, I do advise you to provide your cousin with a written invitation, preferably with a large stamp (especially if your cousin lives in Western Ukraine). For some reason, Ukrainian passport control officers at Kyiv airport automatically suspect everyone from L'viv region in having obtained false foreign passport, and "purchased" US visa. This is when your invitation will come very handy!
For your cousin a word of advice: if he still works, it would be a good idea to get a letter from his place of work (just a short positive reference about him being an honest, working person, and if possible a guarantee that he will be coming back). It might sound funny, but it helps. Plus, of course, copies (or originals) of children's birth certificates (to prove that he has dependents he can't leave behind). And, of course, any documents about real estate property in Ukraine (apartment, land, etc.), or personal business.
It is possible to obtain an application form for an interview from American Embassy in Kyiv through the Internet. Your cousin will have to print it out (or have someone do it for him), fill it out, and send it to the Embassy. The answer will come in about 10 days, with the date abd time of interview.
