Hmm...
I surprised that you find this little phrase a stumbling block.
I have 2 in mind.
One I revere and the other one is suspect.
In the case of people like my mother, who is a "matushka" and those who might be called "Dobrodijka" then their time is spent totally in "volunteer" mode and whatever they "make" (as professionals) is only through donations and sometimes the donations is not to them but to the organization and/or institution which provides the help needed.
The suspect volunteer is the one who is in the midst of donating their time knowing full well that their time is spent to gain some thing monetary for this engagement (if not immediately then definitely at a later date). In particular, those who work within some charitable event and then it comes to note that not 100% of the donation is going to the immediate source but a good parcel is going to pay of those so called volunteers.
