Actually, I think the point of the explanation was to say that in cases where some Ukrainians use this format for addressing:

selo Majdan Verkhnij, Ivano-Frankivs'ka oblast

In this case, it is not necessary to translate the words "selo" to "village" and "oblast" to "province." The transliteration from the cyrillic "село" and "область" is all that is necessary since the form must be filled out using latynka. For purposes of clarity it doesn't hurt to include the words "selo", "misto" and/or "oblast", and we offer the transliterations for those words. However, I don't think the appendix says they "must" be included.
