You can go to Ukraine with your old passport with a maiden name. Do not tell anybody when you are leaving Ukraine that you are married to an American, because then they can ask for an immigration stamp from OVIR office. Basically if you leave Ukraine to live in US permanently, you shoul have this OVIR stamp in your pasport. If you do not want to deal with OVIR while you are in Ukraine, just cross the border to Poland or some other country saying that you are just travelling for pleasure, and then can fly out to US from Poland with your green card. Otherwise, you need to get tons of papers while in Ukraine to get your OVIR stamp, and then get a registration stamp from Ukrainian Embassy. Hope this helps.
