Please write to me directly. What you are asking does not make sense. There are no petitioners for DV visa. There are only applicants. There can be a principal applicant and derivative applicants.
I don't know about which forms you are asking.
This type of question is too specific for me to answer it on the board. If you want, I can give you my telephone number when you write to me. then, maybe it will be easier to clarify. I speak Ukrainian.
