That depends on your citizenship status. If you are a citizen of Ukraine, you probably don't need anything - my parents were traveling between here and Lviv with no Polish vis, these days Poland lifted visa requirements for Ukrainians. If you are an American - I honestly don't know but here is the phone number of the Polish Embassy here in Washington DC: (202) 234 3800, give them a call!
