You need to talk to a doctor. The physical surgeon who will administer the test will know these things and will be able to explain them.
The blood test is to check for the presence of HIV antibodies -- and maybe other diseases. Re the X-ray -- you may not need one if your TB skin test was negative. If the TB skin test (I know, it has another name, but I don't know what it is) was positive b/c you had been given a TB vaccine as a child, if you can get medical proof of this, you may still not need an X-ray.
Various vaccinations are required, but these may be waived if "medically contraindicated," i.e. if the doctor decides any of them are not good for you in your medical condition.
You need to tell the civil surgeon that you are pregnant, and s/he will let you know how best to proceed. From my experience, it is unlikely that you will not be able to get adjusted to permanent residence because of your pregnancy.
There is another issue, however. If you are pregnant, you will need to show that you have sufficient income to support an additional person.
