Any letter would probably do. Put the worf "Affidavit" at the top then state something like "To whom it may comcern. I, undersigned..... hereby state that I personally know these people (give your names) living together as a married couple sharing same house and having common finances (speaking of which a joint account would be nice!if you don't have one - open immediately and show a bank statement as a proof). Blah-blah-blah..." The person writing this letter has to sign it in front of a notary public and have his/her signature legalized (can be done in any bank). Other proofs could be having same car insurance, that is you and your husband being covered by the same policy. Anything showing your two names together would do.
