Good question. There doesn't seem to be any hard and fast rule about this since you should be able to explain that there was a change in names.
However, off-hand I would say you're safest writing down the place names AS THEY ARE RECORDED ON YOUR BIRTH CERTIFICATE (i.e., use the original names in transliterated form). If you get called for an interview, you will be required to offer confirmation of the statements you made on the application by comparing whatever documention you provide. If the information differs, then the interviewing officer may choose to disqualify you based on the inconsistency.
