If you are planning to travel to Ukraine, you MUST register at the nearest Ukrainian consulate. This procedure is not difficult, and takes about 6 weeks (assuming you left Ukraine officially "for good", after completing the procedure "vyizdu na postijne prozhyvannia za kordonom). You call the consulate, and request the form. Then, you send the form with all the right papers (green card photocopy, your passport, etc.) In about 6 weeks you get your passport back with the stamp of Consulate General of Ukraine.
It is very important to have this stamp if you want to travel to Ukraine. Without the stamp you will have EXTREME difficulties leaving Ukraine. It might cost you hundreds of dollars to get out, and weeks of anxiety and humiliation.
If you left Ukraine on tourist visa, and stayed, it will be more complicated to get the stamp. But not impossible (it will take you about 6 month)
If you definitely do not want to register with Ukraine consulate, I advise you to travel to Ukraine with a US travel document. It takes couple of month to get it, and you will need to obtain a visa from Ukrainian consulate. Beleive me, it is easier to register.
