I recently applied for visa from Ukrainian consulate in Chicago. They sent back my passport, and returned my money order. They sent letter with package stating visa not required for Americans travelling to Ukraine, if the stay is less than 90 days in a 6 month period. It is the same policy and the Canadian waiver and now the EU waiver which is now the same as US and Canada. So no visa for Americans required.
