I was in U.S. and I submitted my new photo to be put in a domestic passpost 9 months past my 25th birthday. No questions were asked. No penalties.
I am not aware of any per day penalties. It may be a fixed penalty. Still, I don't think your wife will be penalized. She has a legitimate reason for late submission. She wasn't physically present in Ukraine, and Ukrainian consulates abroad doesn't deal with Ukrainian domestic passports.
