Hi... you will need to send this request to UA consulate along with a money order and your passport. Your child would enter Ukraine and leave Ukraine, on your Ukrainian passport. When you arrive to the US, have your green card ready along with a passport for you and for the child, you'll need US passport.You do not need any stamps to enter back to US.
P.S. If 2 parents both have UA passport, and only 1 have your dauther added to the passport, only that parent can bring in and outof the UA.
I have alot of experience, i only do it 5-6 times a year(ha-ha)
