Could you please give some reference or write where do I need to file this request to include our daughter to my Ukrainian passport?
And do I understand the procedure right:
- while going to Ukraine we show her name in our passport
- while going to USA we present her US passport
Is it ok that there will be no stamps about leaving the US in her passport and only stamps about entering?

: Hi Val... I have 2 kids and I have added them to my passport. You need a written request to add them to your passport. It also needs to be signed by the father of your child. I believe it would cost you $10, plus the pre-paid envelop for return. When your child turns 5 years old, you we'll need to add her photo, until then.....enjoy your trip
