Hi Val... I have 2 kids and I have added them to my passport. You need a written request to add them to your passport. It also needs to be signed by the father of your child. I believe it would cost you $10, plus the pre-paid envelop for return. When your child turns 5 years old, you we'll need to add her photo, until then.....enjoy your trip
