Try to get a visa through any of the tourist agencies. My sister never tried to get a 5 year one.
There has been an advertisement even at this site about Ukrainian visas. Just call them - sometimes spending few bucks is worth sparing your time and avoiding that specific atmosphere( at least in NYC Ukrainian consulate).
