In case you can't read the cyrillic font, what Vasyl is saying is that you may use the address in the US for your mother's application, but make sure that the same address appears on both the envelope and on the application inside. The instructions are pretty clear as to how to set up the form and envelope if the mailing address is in the US. Please read through the instructions and explanations carefully.
