The recommended posture is only that it be a head shot (from the neck up, that is) and full face - a passport photo bureau photo would be perfect.

The exact placement of the photo is indicated on the sample form that is provided on this website.

By clear tape, they mean for example not "duct tape" or "electrical tape", both of which are opaque and not clear. Clear tape is the ordinary Scotch tape that is commonly used in the US. You know it is clear if you apply it to tape the picture to the paper, and you can see that part of the picture that is covered by the tape even through the tape.

In the sample form, you will see that the person's head is partially covered by the "clear tape", yet you can see the entire head.
