There are several books about names listed in the 2000 Yevshan catalog, bottom of p.60(request a free catalog at www.yevshan.com or register to see their on-line catalog. Two books are in Ukrainian, but one is in English. It is called Ukrainian Christian Names by Roma Gauk. Quote from the catalog "The object of this booklet is to provide a ready reference to more common names with their English equivalents and direct translations. Describes the annointing with holy oil, and the Act of Baptism, the Washing, the Shearing and other associated rituals. Also touches upon the use of diminuitives and historical infor. 31 pp. paper Canada." I recently received a copy of this small booklet. I think it has what you are looking for.
